I hoped to have time for some short takes, but when I return from my Prosthetist, I have a medical mess to clean up. Between my Ocular Oncologist, my Radiation Oncologist, and my substitute Primary Care doctor, they neglected to get the referral paperwork filled out for the Radiation Oncologist to make the plaque that was inserted behind my eye, so my claim was denied. Now I have to figure out who needs to do what to correct their errors. ARGH!!

Sometimes when you get a "Denial of Service", just resubmitting the claim can resolve it. You know, paperwork snafus and such.
But here are two links that might prove helpful. The first is sort of an overview, and the second is a worksheet (WS) to keep track of tthe process – although I doubt yours will be that cumbersome.

It's so nice to have someone around who understands medical mayhem paperwork (and I'm sure his proposed loo for the GOP campaign is no coincidence there.) It's too bad you have to go theough the process, though. Don't worry, we'll still be here, take care of yourself.

While it is better to have to deal with medical paperwork glitches than with actual medical glitches or medical issues as such, this sounds so terribly frustrating, TomCat. America has the most expensive healthcare system in the world and yet things like this appear to happen all the time. It's about time Americans got their money worth in medical care and service. In a single payer system it wouldn't be your job to correct the errors that are made within the system, but of those those who are responsible for making it. But you haven't got it (yet), so be sure keep your cool and have those that messed up take responsibility for their mistake. Good luck.
Insurance company glitches, and paperwork are some of the banes of a civilized(?) society. Man, you get hit from every frigging direction, don't you?
